چکیده

S ARCOIDOSIS is considered to be a granulomatous process that affects multiple systems. Neurologic symptoms develop in 1% to 2% of cases with Boeck’s sarcoid.’ Although the neurologic presentation has been uncommon, granulomas have been observed in up to 14% of patients with sarcoid at autopsy.’ The purpose of this presentation is to demonstrate the protean manifestations of sarcoidosis within the brain and its coverings.

Neurosarcoidosis

Neurosarcoidosis is an uncommon but potentially serious manifestation of sarcoidosis. While the cranial nerves are most frequently affected, neurosarcoidosis can involve other nervous system tissues including the meninges, brain parenchyma (especially the hypothalamic region), spinal cord, peripheral nerve, and muscle. Diagnosis may be particularly challenging when neurosarcoidosis occurs in is...

Common variable immunodeficiency with coexisting central nervous system sarcoidosis: case report and literature review with implications for diagnosis and pathogenesis.

We describe a patient with a history of longstanding primary generalised epilepsy, on anticonvulsant therapy, who presented with fever, headache, worsening seizures and hallucinations. Among various investigations, the patient had high CSF protein and ACE levels, leptomeningeal nodular enhancement on MRI brain and non-caseating granulomas in the brain and meninges on the biopsy. The patient was...

CHAPTER 11 Neurosarcoidosis

Sarcoidosis is an inflammatory multisystemic disorder. Its cause is not known. The disease may involve any part of the nervous system. The incidence of clinical involvement of the nervous system in a sarcoidosis population is estimated to be y5–15% [1, 2]. However, the incidence of subclinical neurosarcoidosis may be much higher [3, 4]. Necropsy studies suggest that ante mortem diagnosis is mad...

Necrotizing neurosarcoidosis masquerading as a left optic nerve meningioma: case report.

Isolated neurosarcoidosis involving the optic nerve meninges is extremely rare and is often indistinguishable from a meningioma in its anatomic site and MR imaging presentation. Characteristic findings include enhanced perineural encasement and thickening of the affected optic nerve on contrast-enhanced T1-weighted cranial MR imaging studies. We present the case report of a patient with isolate...

Sarcoid tumour: continuing diagnostic problems in the MRI era.

The eVects of sarcoidosis on the nervous system are protean. They include involvement of the meninges (cranial nerve palsies, pituitary, and hypothalamic eVects), peripheral nerves (symmetric polyneuropathy, multifocal neuropathy), and brain parenchyma (lesions of brain and spinal cord without obvious meningeal involvement); and also secondary effects of systemic disease, such as intracerebral ...
